For community organizations that want to help their families meet the challenges of aging at home, Community Aging Circles (CAC) provides a platform with tools, training, and support so communities can surround older people and caregivers with trusted help

What We Do

One in four American adults is now a family caregiver. Most are navigating complex care needs alone.

Community Aging Circles (CAC) is based on the premise that it takes a community to make aging in the community work. We are dedicated to empowering community organizations to provide their caregiving member families  the tools, support, and connections needed to help older and vulnerable individuals age safely at home—and to connect these caregiving members to the people who matter most, other caregivers.

Because care works best when it’s shared.

How CAC Helps

CAC enables community organizations to give families practical ways to care for an older person at home. Families can use CAC’s support to bring relatives, friends, neighbors, volunteers, and service providers into a shared circle of care, organize tasks and schedules, exchange updates, and delegate responsibilities so the work of caregiving does not fall on one person alone, CAC:

  • Provide stechnology, training, and support that help caregivers organize resources and share responsibilities.
  • Helps families bring relatives, friends, neighbors, volunteers, and service providers into a coordinated circle of care.
  • Connects caregiving community members so they can share information, resources, and encouragement.
  • Makes these supports available at no cost to families or community organization

CAC’s work grows out of both community need and the experience of leaders who have spent decades helping older adults, families, and caregivers. The people behind CAC bring together nonprofit leadership, aging-services experience, technology expertise, community development and a shared commitment to making caregiving less isolating and more coordinated.

How It Works

Build a “Circle of Care” Around Every Individual

At the heart of our model is the Circle of Care—a coordinated network of people and services supporting one individual.

Step 1: Connect

We partner with community organizations to identify and support families in need.

Step 2: Organize

Families create a personalized care circle that may include: Family members Friends and neighbors Healthcare providers Community services (e.g. meals, transport, home aides)

Step 3: Coordinate

Using easy-to-use digital tools, everyone in the circle can: Communicate in one place Track care tasks and responsibilities Share updates in real time

Step 4: Support

CAC provides training, resources, and ongoing guidance to ensure families feel confident and supported.
